Every patient visiting our office and receiving care will have several documents to review and sign. These will include policies on risks of care, how we will protect your privacy, protecting your health information, how to manage appointment times, cost of care, taking advantage of insurance and our fee schedule.
These three forms are signed by all visitors. Informed consent, privacy practices and our cancellation. We encourage you to review these documents and ask any questions you may have.

We are responsible for protecting your health information, here we ask in which way we may communicate with you out side of the office.

We ask for a 12 hour notice if you must change or cancel appointments.

The next set of policies covers cost of care, which there are three basic ways to cover your cost: pay cash for your visit, bill your insurance company, or having a workers compensation claim or auto accident claim. Individuals over the age of 65 will be required to review and sign an advanced beneficiary notice for medicare, regardless if you wish to bill medicare.

Unbroken body is not a co-pay only office. Chiropractic care is limited to certain covered services. We collect for non-covered services at every time of service.

Medicare cost of care is one low flat rate fee and is described in the Advanced Beneficiary Notice

We have several options for you if you are involved in an auto accident, workers comp injury.

Included here is our 2024 fee schedule which reflects the cost of services that you will see on your explanation of benefits. These reflect the costs we bill, not the cost of services to the patient.
