Office forms
Every patient visiting our office and receiving care will have several documents to review and sign. These will include policies on risks of care, how we will protect your privacy, protecting your health information, how to manage appointment times, cost of care, taking advantage of insurance and our fee schedule.
Patient care and protection policies. These three forms are signed by all visitors. Informed consent, privacy practices and our appointment cancellation policy. We encourage you to review these documents and ask any questions you may have.
The next set of policies covers cost of care, which there are three basic ways to cover your cost: pay cash for your visit, bill your insurance company, or having a workers compensation claim or auto accident claim. Individuals over the age of 65 will be required to review and sign an advanced beneficiary notice for medicare, regardless if you wish to bill medicare.
Assistance with Insurance Coverage
Here we have included links to the Department of Health Care Services and the Department of Managed Health Care. Department of Health Care Services helps individuals find medical care and services. Department of Managed Health Care helps individuals manage disputes with their insurance companies, such as insurance companies not paying in full for services when they promise to do so.
